| 过期妊娠 prolonged pregnancy 达到或超过预产期2周以上尚未临产的妊娠 。 可因内分泌因素或遗传因素引起。过期妊娠可出现两种情况：①胎盘功能正常，不影响胎儿健康，但分娩时常因胎儿过大，胎头过硬造成难产。②胎盘功能减退，胎儿的氧气供给与营养不足，胎儿发育受影响，死亡率增高。诊断应首先确定妊娠是否过期，核实预产期（见产前检查），可疑时行超声检查 。若超声检查提示胎儿足月而羊水量少或胎盘老化均有助诊断，进一步检查胎盘功能。若妊娠确已过期，或有胎盘功能减退者，应终止妊娠，根据宫颈条件给予人工引产或剖宫产结束分娩。 |
